Alimentación lenta versus rápida en pacientes prematuros de muy bajo peso al nacimiento.

- Abstract
Objetivo. Determinar si el volumen de inicio y la velocidad de avance a 30 mL/kg/día en la alimentación afecta la incidencia de enterocolitis necrosante (ECN) en recién nacidos prematuros de muy bajo peso. Material y métodos. Ensayo clínico experimental, prospectivo, controlado y aleatorizado. Se estudiaron 80 prematuros de 501-1500 g. Fueron aleatorizados en dos grupos: A(n =40) alimentación rápida con inicio de 30 mL/kg/día con incrementos de 30 mL/kg/día. B (n =40) alimentación lenta con inicio de 15 mL/kg/día con incrementos diarios de 15 mL/kg/día, hasta alcanzar 160 mL/kg/día. Resultados. La incidencia de ECN (estadios de Bell >0 =11) fue igual en ambos grupos. Un paciente en cada uno (2.5%). La alimentación fue iniciada a una edad postnatal similar en ambos grupos; media de edad 2.7 días grupo control y 2.78 grupo de estudio (p no significativa). Se observó diferencia significativa en los días en los dos grupos al alcanzar la alimentación tope máxima: lento 10.8, y rápido 6.42, p <0.001. Se encontró mayor número de pacientes con residuo en el grupo de estudio, sin embargo, no hubo diferencia en el porcentaje del residuo significativo. Conclusiones. La alimentación rápida en prematuros con inicio e incrementos de 30 mL/kg/día es bien tolerada. Existe alcance más rápido del tope de alimentación máxima. La alimentación rápida no incrementa la incidencia de ECN.
