- Abstract
Introducción. El íleo meconial es una obstrucción intestinal producida por un meconio denso y espeso. El íleo meconial complicado es aquel que presenta vólvulo, atresia y/o peritonitis meconial in utero. La mortalidad de la peritonitis meconial al momento del nacimiento es de 50 a 60%; 90% de los pacientes con este trastorno padecen fibrosis quística. Las calcificaciones en la radiografía simple sugieren peritonitis meconial. La búsqueda de mutaciones por biología molecular suele hacer el diagnóstico en 95% de los pacientes y el diagnóstico definitivo se logra al realizar electrolitos en sudor. Caso clínico. Masculino, hijo de madre de 41 años, gesta 2, aborto previo. En el embarazo actual a las 28 semanas de gestación acudió a consulta por edema de miembros pélvicos y actividad uterina. Se detectó hidrops fetalis y sufrimiento fetal agudo; decidiéndose terminar embarazo por cesárea. Se efectuó reanimación y masaje cardiaco, se intubó y pasó a terapia neonatal; Apgar 4/6/8. Se realizó paracentesis por restricción respiratoria obteniendo líquido amarillento cristalino. Peso 1 725 g y talla 36.5 cm. Exploración física: edema subcutáneo en cabeza y extremidades, campos pulmonares hipoventilados, abdomen globoso a expensas de líquido en cavidad peritoneal. Por síndrome de dificultad respiratoria se aplicó surfactante. Al tercer día presentaba conducto arterioso persistente, con falla al intento del cierre farmacológico, por lo que se realizó cierre quirúrgico sin complicaciones al séptimo día, con lo que mejoró ventilatoria y hemodinámicamente. Al noveno día se practicó laparotomía exploradora hallando quiste meconial gigante con perforación intestinal en íleo a 20 cm de la válvula ileocecal, se realizó ileostomía y fístula mucosa. En el postquirúrgico continuó con apoyo ventilatorio, parenteral y antibioticoterapia. Actualmente se encuentra tolerando alimentación enteral. Prueba molecular para fibrosis quística negativa. Conclusiones. La mortalidad de la peritonitis meconial puede disminuirse con el apoyo intensivo neonatal y la intervención quirúrgica oportuna. En nuestro paciente se manejó la insuficiencia respiratoria, se resolvió el problema hemodinámico que dio el conducto arterioso y se dio apoyo nutricional parenteral total temprano. En el tratamiento quirúrgico de la peritonitis meconial es fundamental estratificar las prioridades que ponen en peligro la vida en forma inmediata como se hizo en este caso. El paciente cayó dentro de 10% de los casos con peritonitis meconial sin fibrosis quística; la prueba molecular detecta 95% de los casos pero aún es necesario realizar la ontoforesis.
