Systematic evaluation and Meta-analysis of energy expenditure in children with congenital heart disease.

- Abstract
Objective: To systematically review and quantitatively analyze the energy expenditure in children with congenital heart disease(CHD). Methods: The Cochrane Library, JBI Library, MEDLINE, EMBASE, Scopus Database, ProQuest and EBSCO and Chinese Biomedical Literature Database, Chinese Knowledge Network, Wan-fang Data Resource System and Chinese Science and Technology Journals Database were searched by computer from building up to December 2015. Meta-analysis of total energy expenditure(TEE) and resting energy expenditure(REE) were performed before and after operation, and the heterogeneity analysis was performed by RevMan 5.3 software. The data were analyzed with AHRQ in cross-sectional study. And according to the results, the corresponding effect model was used to merge the effect value. Results: Twelve papers were included in the systematic review and nine in the meta-analysis. All of them were prospective study with healthy control group. There were four papers reported the use of doubly labelled water to measure preoperative TEE, and the Meta-analysis showed that incorporative WMD was 21. 51[95%CI(17.61,25.41)], which was 21.51 kcal/(kg⋅d) higher in preoperative TEE than in healthy children, D). Two papers reported that postoperative TEE did not undergo Meta-synthesis because of different dosage units. Four papers reported the use of a nutritional metabolic test system to measure preoperative REE,and a randomized effect model analysis showed incorporative WMD was 4.32 [95%CI(-1.20,9.85)]. Three papers reported postoperative REE, and a randomized effect model analysis showed incorporative WMD=7.80[95%CI(-9.17,24.77)]. Conclusions: Infant children with congenital heart disease had higher total preoperative energy expenditure compared with healthy children;there was no difference in resting energy expenditure between preoperative and postoperative in comparison with healthy children.
