- Abstract
Schwere Traumata des Auges mit Beteiligung des posterioren Segments kommen häufig vor, und ihre ophthalmologische Versorgung stellt trotz der umfassenden technischen Fortschritte der letzten Jahre weiterhin eine erhebliche Herausforderung dar. Eine dieser Herausforderungen liegt darin, den optimalen Zeitpunkt für die endgültige Rekonstruktion einschließlich Vitrektomie zu bestimmen. Während recht klar auf der Hand liegt, dass der Nahtverschluss der Wunde bei einer offenen Bulbusverletzung schnellstmöglich durchzuführen ist, ist weniger klar, ob die Vitrektomie bei demselben Eingriff erfolgen sollte (primäre umfassende Rekonstruktion) oder zu einem späteren Zeitpunkt (mehrzeitiges Vorgehen) - und wenn, wie viel später. Im vorliegenden Bericht werden 4 Optionen für das mehrzeitige Vorgehen gezeigt: früh (Tag 2-4), verzögert (Tag 5-7), spät (Tag 8-14) und sehr spät (mehr als 2 Wochen). Je früher die Vitrektomie durchgeführt wird, desto höher ist das Risiko für intraoperative Komplikationen. Je später hingegen die Vitrektomie erfolgt, desto höher sind Inzidenz und Schwere von postoperativen Komplikationen, deren schwerwiegendste die proliferative Vitreoretinopathie ist.
