ALTERAÇÕES LABORATORIAIS EM CÃO INFECTADO POR TRYPANOSOMA EVANSI.

- Abstract
Dentre os principais hemoprotozoários, o Trypanosoma evansi tem importância mundial por sua ampla distribuição. A infecção ocorre, principalmente, durante o repasto sanguíneo de insetos pertencentes ao gênero Tabanus, Stomoxys, Haematopota e Lyperosia. Os pacientes acometidos podem apresentar anemia, leucopenia com neutropenia, aumento de creatinina, ALT, AST e PT, hipoalbuminemia e diminuição na relação albumina/globulinas. Os métodos diagnósticos mais utilizados são RIFI, ELISA, PCR e detecção do parasita no esfregaço sanguíneo. Sendo a tripanossomíase uma patologia pouco diagnosticada, torna-se importante relatar os achados laboratoriais desse caso em cão. No dia 14/03/2023 foi atendido um cão, raça americano, 3 meses, apresentando apatia, diarreia, vômito, leucocoria, adipsia e anorexia. O hemograma revelou plasma ictérico, anemia macrocítica hipercrômica, leucopenia com eosinopenia e neutropenia, trombocitopenia, e inúmeras formas extracelulares flageladas tripomastigotas (setas) compatíveis morfologicamente com Trypanosoma evansi, pela ausência de cinetoplasto. Nas análises bioquímicoséricas detectou-se 1,9 mg/dL de creatinina (0,5 - 1,5 mg/dL) e 207 U/L de fosfatase alcalina (20 - 150 U/L) como alterações. Foi iniciado o tratamento para tripanossomíase, porém, houve piora do quadro, sendo realizada eutanásia do paciente 5 meses após o diagnóstico. Os resultados encontrados no hemograma justificam-se em razão da elevada carga parasitária e consequente formação de grandes quantidades de imunocomplexos circulantes, que se depositam na membrana de eritrócitos, leucócitos e plaquetas, promovendo a destruição intravascular dos mesmos. A hemólise intravascular não é comum em infecções por Trypanosoma spp., porém, em infecções agudas a anemia hemolítica aguda pode ocorrer pelo mecanismo acima mencionado. A elevação da creatinina pode ocorrer pela desidratação por diarreia, vômito e adipsia relatados. A fosfatase alcalina é elevada em filhotes devido ao crescimento ósseo. O relato das alterações laboratoriais encontradas são inespecíficos e deve-se considerar a tripanossomíase como diagnóstico diferencial na instituição de um tratamento específico e eficaz.
