CRISTALÚRIA E MICROLITÍASE EM CANINO DA RAÇA DÁLMATA - RELATO DE CASO.

- Abstract
Foi atendido em uma clínica, um canino, macho, da raça Dálmata, 10 meses de idade, com queixa de excessiva concentração urinária, durante três episódios consecutivos. A urina apresentava-se espessa, cor amarelo forte, aspecto arenoso e presença de secreção em região peniana. Conforme tutor, paciente não apresentava disúria ou polaciúria, ingesta hídrica e alimentar adequadas, sem apresentar qualquer alteração clínica. Foram coletadas amostras de sangue, para hemograma e bioquímicos (Albumina, ALT e Creatinina), e urina por micção espontânea, para urinálise. Hemograma e bioquímicos não demonstraram alteração. A urinálise evidenciou turbidez acentuada, cor amarelo escuro, discreta bacteriúria, além de cristalúria significativa, com observação de cristais de oxalato de cálcio, urato amorfo e ácido úrico. A urina apresentava-se ácida (pH=5,0), com densidade de 1,046 e traços de proteína. À ultrassonografia, visualizou-se, em região de pelve renal, sedimento ecogênico bilateral, além de pontos hiperecogênicos em bexiga, achados compatíveis com microlitíase e cristais. Novas amostras de sangue e urina foram submetidas a análises complementares, incluindo mensuração de ácido úrico e bilirrubina sérica, sem nenhuma alteração nos valores obtidos, permanecendo a urina com a concentração excessiva de cristais e mesma apresentação físico-química. A intensa cristalúria deste paciente pode ter predisposto a formação de microurólitos. A formação dos cristais se deve a diversos fatores como o pH da urina, dieta inadequada, podendo inclusive ser encontrados em urinas de animais saudáveis. Além disso, o fator genético em Dálmatas predispõe a formação de urólitos, pois os mesmos excretam ácido úrico na urina como produto final do metabolismo das purinas, tendo capacidade reduzida de oxidação do ácido úrico em alantoína, quando comparados a cães de outras raças. Todos os Dálmatas são naturalmente hiperuricosúricos, favorecendo a ocorrência de urolitíase, que em geral ocorre dos 3 aos 6 anos de idade, apresentando precocidade no canino deste relato. A hiperuricosúria sem hiperuricemia pode ser explicada pela impermeabilidade de membranas celulares glomerulares ao ácido úrico, prejudicando a reabsorção nos túbulos proximais renais de Dálmatas. Definiu-se, clinicamente, proceder com tratamento e acompanhamento nutricional adequado deste paciente, com especialista, para reverter a formação de cristais e evitar a demasiada concentração urinária.
