- Abstract
In this presentation, we wanted to emphasize the importance of phaeochromocytoma in the differential diagnosis of the patients experiencing intraoperative sudden hypertension and tachycardia episodes. A 19 year old male patient who was operated on with the diagnosis of left urolithiasis, experienced intraoperative sudden and serious hypertension attack along with tachycardia, flushing and acute pulmonary edema. Systolic arterial pressure was 270/190 mmHg and the heart rate was 170 beats/min. Patient was treated with nitroglycerine infusion and IV propranolol without any answer to the regimen and then managed with Na-nitroprussid and esmolol infusion. After the regulation of the hemodynamic parameters, post-operative care of the patient continued in the intensive care unit. Patient was referred to another clinic for the treatment of phaeochromocytoma after his vital signs had been stabilized.In case of sudden intraoperative hypertension and tachycardia episodes, phaeochromocytoma should be considered as one of the possible etiological factors in the differential diagnosis. Rapid and accurate diagnosis of this situation can decrease the mortality and morbidite of the cases. Undiagnosed phaeochromocytoma cases can be operated on for different surgical procedures rather than this situation and can still cause great difficulty for the anesthesiologist managing the patient. If the diagnosis is rapid and accurate, the answer to the treatment with α-blocker agent is adequate.
