The article discusses the barriers to the use of rapid transcranial magnetic stimulation (rTMS) and electroconvulsive therapy (ECT) for the management of treatment-resistant depression. Topics mentioned include the concept of stepped psychiatric care, the clinical effectiveness of rTMS over ECT, the lack of access to psychiatric care and funds for modern equipment, and the logistical challenges in providing rTMS and ECT to outpatients.
