Résumé: Une étude prospective a évalué la performance de l'échographie abdominale en comparaison à l'entéro-IRM avant d'initier une biothérapie. Chez 21 patients, il existait une très forte corrélation entre l'échographie et l'endoscopie et entre l'échographie et l'entéro-IRM concernant la localisation du segment le plus atteint. Un cas clinique issu d'une série de cas a décrit l'efficacité du tofacitinib chez une patiente ayant une colite aiguë sévère hospitalisée en échec de deux anti-TNF, d'un traitement par ciclosporine relayé par le védolizumab. Ce traitement a la particularité d'avoir une action rapide et ainsi pourrait avoir une utilité chez les patients ayant une colite aiguë sévère. La survie sans colectomie à six mois de 55 patients était de 73,6 %. Un Projet médico-soignant partagé de la filière Hépato-Gastro-Entérologie du GHT d'Eure-et-Loir a été mis en place pour la prise en charge des maladies inflammatoires chroniques de l'intestin, articulé autour de la création d'un poste d'IDE de coordination MICI. L'intervention de l'IDE a été jugée utile ou très utile par 87 % des patients. A prospective study evaluated the performance of abdominal ultrasound compared to entero-MRI before initiating biotherapy. In 21 patients, there was a very high correlation between ultrasound and endoscopy and between ultrasound and MRI enterography regarding the location of the most affected segment. A case report from a case series described the efficacy of tofacitinib in a patient with severe acute colitis hospitalised after failure of two anti-TNF agents, ciclosporin treatment and vedolizumab. This treatment has the particularity of being fast acting and thus could be useful in patients with severe acute colitis. The 6-month colectomy-free survival of 55 patients was 73.6%. A shared medical and nursing project for the Hepato-Gastro-Enterology branch of the Eure-et-Loir GHT has been set up for the management of chronic inflammatory bowel diseases, based on the creation of a position for a coordination nurse for IBD. The intervention of the nurse was considered useful or very useful by 87% of patients.
