RETROPERITONEAL APPROACH FOR AORTIC SURGERY.

- Abstract
The retroperitoneal approach to aortic surgery, as first described by Rob, was used in 75 consecutive cases of aortic resection, aortoiliac, aortofemoral and iliopoliteal by-pass procedures and celiac and renal artery angioplasty. The patients ranged in age from 47 to 82 years. There were three postoperative deaths in the series. Postoperative complications occurred in 11 patients. There was no wound dehiscence, no instance of incisional hernia, and the need for tracheostomy or gastrostomy was eliminated. The average hospital stay was 9 days. In conclusion, we believe this approach to be a safe procedure for older individuals, for poor risk patients and those with emphysema and cardiovascular disease. The incidence of postoperative complications is greatly reduced. Also, as Rob pointed out, there is less postoperative pain and a shorter convalescence. Although we used the right-sided approach in only two cases, we believe this may well be indicated, and indeed better, for unilateral lesions of the right iliac and femoral arteries.
