When treating a Class II subdivision malocclusion using a conventional multibracket appliance, it is generally necessary to extract three to four premolars to correct the unilateral Class II occlusion and the deviation of the midline between the maxillary and mandibular incisors. However, when using the Herbst appliance, an asymmetric advancement of the mandible (asymmetric bite jumping) will effectively correct both the unilateral Class II malocclusion and the midline deviation without the need for tooth extractions.
