- Abstract
A miosite infecciosa pode ser causada por um amplo espectro de agentes. Uma de suas formas é a piomiosite. Uma potra de 2 anos, da raça Crioulo, foi encaminhada ao Hospital Veterinário da USP/Pirassununga com queixa de desconforto abdominal. Na anamnese, foi informado que os sinais começaram naquele mesmo dia e que o animal estava sob treinamento intenso e sobrecarga de exercícios. Ao chegar, o paciente apresentava taquicardia, taquipneia hipertermia, hipomotilidade intestinal, mucosas congestas e TPC aumentado. Realizou-se hemograma completo, cuja principal alteração era aumento de fibrinogênio (1200). Inicialmente instituiu-se fluidoterapia e administrou-se antifisético com o intuito de tratar os sinais de abdômen agudo, com melhora em 24 horas. Apesar da melhora na queixa inicial, o animal apresentou episódios de diarreia e febre nos dias subsequentes, sendo iniciada antibioticoterapia com gentamicina (6,6 mg/kg SID IV), cefitiofur (4,4 mg/kg SID IV) e flunixin meglumine (0,25 mg/kg TID IV). No oitavo dia do tratamento, o animal apresentou inchaço no membro pélvico esquerdo, com presença de pulso digital. Ao repetir os exames laboratoriais foram constatados aumento de ALT, fosfatase alcalina, proteínas plasmáticas e globulinas, anemia, neutrofilia com desvio à esquerda regenerativo e presença de linfócitos reativos. Com isso, os antibióticos foram substituídos por doxiciclina (dose 10 mg/kg BID VO) e metronidazol (15 mg/kg TID IV), juntamente a Diuzon® (três administrações de 10 ml) e DMSO (0,3 mg/kg BID IV). Após 20 dias de evolução, observou-se que o animal havia perdido a capacidade de defecar e urinar por conta própria, precisando de sondagem e auxílio para a realização destas funções e poucos dias depois evoluiu para decúbito. Realizou-se análise ultrassonográfica do membro pélvico esquerdo do animal, que mostrou várias porções de conteúdo anecoico com pontos hiperecoicos, sendo decidido fazer a incisão para drenagem deste conteúdo. Ao ser acessado, retirou-se grande quantidade de conteúdo purulento, o qual foi enviado para o laboratório para cultura, sendo positivo para Staphylococcus aureus. Devido à evolução negativa do caso, optou-se pela eutanásia do animal. A piomiosite é definida como uma infecção bacteriana aguda intramuscular, secundária à migração hematógena da bactéria em questão. Atletas jovens podem ter maiores riscos de desenvolver tal quadro devido a lesões de sobrecarga com epsódios transitórios e assintomáticos de bacteremia. Na necropsia, constatou-se extensa lesão necrosante da muscultura em membro pélvico esquerdo. As raízes de inervação de região sacral tambem haviam sido acometidas, assim como pelve e início da musculatura em membro pélvico direito. Conclui-se que o acometimento de grupos musculares extensos prolongam o tempo de tratamento necessário do quadro e pioram o prognóstico do animal.
