- Abstract
O insulto térmico testicular afeta negativamente a espermatogênese e é uma causa comum de degeneração testicular. As alterações na qualidade do sêmen após o estresse térmico são bem compreendidas, mas seus efeitos na função das células de Sertoli ainda não estão bem caracterizados. Novas técnicas permitiram o isolamento e análise da inibina-B e observaram uma redução sérica após insulto tóxico. Similarmente, o hormônio antimulleriano apresentou elevação após insulto tóxico com RTI-4587-073. A hipótese neste estudo é que a inibina B e o AMH séricos se correlacionam com os parâmetros espermáticos após a insulação escrotal e poderiam ser utilizados como marcadores de dano testicular e no acompanhamento da evolução do quadro clínico. Garanhões (n = 9) entre 5 e 14 anos, foram divididos em grupos controle (n = 4) e tratado (n = 5). O grupo tratado foi submetido à insulação escrotal por meio de compressa termoelétrica durante 1h, a cada 12h, por dois dias e a temperatura escrotal média durante o procedimento foi de 52 ± 7 oC. A resposta testicular endócrina foi mensurada por avaliação dos compartimentos tubulares (inibina-B e AMH) e intersticial (testosterona e sulfato de estrona). Os parâmetros espermáticos avaliados foram número de espermatozoides totais, defeitos espermáticos, motilidade total (MT) e progressiva (MP) e número de espermatozoides rápidos (RAP). Os resultados hormonais e espermáticos foram comparados semanalmente com os valores pré-tratamento até 90 dias após o tratamento. As concentrações séricas de inibina-B, AMH e sulfato de estrona foram determinadas por kits comerciais de ELISA e a testosterona determinada por radioimunoensaio. Os ejaculados foram colhidos em vagina artificial modelo Botucatu® (Botupharma LTDA®, Botucatu, SP, Brasil) e a cinética espermática analisada pelo sistema CASA (IVOS Version 12 Hamilton Thorne Research, MA, USA). A avaliação da morfologia espermática foi realizada utilizando-se microscópio de contraste de interferência diferencial de fase (DIC)3 e a concentração realizada com câmara de Neubauer espelhada (Optik Labor®, Lancing, Inglaterra). Os dados paramétricos foram avaliados por modelo misto usando Bonferroni para comparação entre os tempos e os dados não paramétricos foram avaliados por Kruskal Wallis. A insulação escrotal reduziu significativamente a MP entre os dias 17 e 45 pós-insulto, enquanto MT e RAP reduziram entre 30 e 36 dias. Os defeitos totais aumentaram no período de 24 a 45 dias pós-insulação escrotal. No entanto os níveis hormonais séricos não apresentaram alteração significativa durante o período avaliado. Em resumo, o insulto térmico escrotal parece afetar as células germinativas e as células de Leydig, mas não as células de Sertoli e, consequentemente, não alteraram a síntese de inibina-B e AMH. Assim, o AMH e a inibina-B parecem não ser biomarcadores confiáveis para dano e recuperação testicular após insultos testiculares térmicos.
