Urolitíase uretral em equino.

- Abstract
A urolitíase em equinos, apesar de rara, é mais comum em vesícula urinária, sendo quadros de envolvimento uretral esporádicos. Também é uma enfermidade mais frequente em animais adultos a idosos com dieta rica em minerais. O objetivo deste trabalho é relatar um caso de urolitíase uretral em fêmea equina. Foi atendido no Hospital Veterinário da Unoesc-Xanxerê/SC um equino, fêmea, de aproximadamente 19 anos de idade, com queixa de incontinência urinária e disúria com três semanas de evolução. A égua já tinha histórico de cistite recorrente, tendo melhora dos sinais com tratamento, mas com recidiva. No exame clínico o animal apresentou taquicardia, mucosas hipocoradas, desidratação, disúria e hematúria. Na hematologia e bioquímica, apresentou anemia, leucocitose por neutrofilia e azotemia, com incremento de creatinina (6,0 mg/dL) e uréia (152 mg/dL). Na palpação transretal a égua apresentou desconforto e, na ultrassonografia, observou-se conteúdo com debris celulares em bexiga. Optou-se, então, por realizar vaginoscopia e uretroscopia, sendo identificada a presença de urolitíase, com urólito de cerca de 10 cm de diâmetro, encarcerado na porção final da uretra, quase a obstruindo por completo. Realizou-se, então, a retirada do mesmo com a paciente em estação, sob sedação a partir de infusão de detomidina (10 µg/kg/h), anestesia local peridural baixa com lidocaína 2% (6 ml) com vasoconstritor e analgesia complementar com butorfanol (25 µg/kg), por via intravenosa. Na manipulação, observou-se aderência do urólito em canal uretral, este com estenose parcial, optando-se pela uretrotomia parcial. Após o procedimento, foram realizadas uretroscopia e cistoscopia, constatando não haver mais cálculos ou lesões vesicais, como lacerações. A uretra apresentou grande área de lesão da mucosa, com múltiplas lacerações, optando-se por não inserir sonda, e realizar lavagem com solução fisiológica e clorexidine a 0,2%, monitorando a cicatrização para evitar estenose. A paciente seguiu sob fluidoterapia de suporte, terapia anti-inflamatória, antimicrobiana e protetor gástrico por sete dias, sob monitoração até completa cicatrização. Realizou-se, também, monitoramento ultrassonográfico de bexiga e rins. Com uma semana de evolução, no perfil hematológico e bioquímico, houve estabilização dos valores de creatinina e ureia, retorno de leucócitos e hematócrito aos valores de referência e urinálise com discreta hematúria, incremento de células descamativas de bexiga e raros cristais de carbonato de cálcio, normalizando os valores. Com isso, a paciente teve alta 14 dias após o procedimento. Nos equinos a ocorrência de urolitíases é rara, podendo ser atribuída à grande quantidade de muco produzido pelas glândulas da pelve renal e do ureter proximal, impedindo aderência dos cristais no urotélio. Contudo sempre torna-se necessária a realização de exames complementares, como urinálise e bioquímica renal, perante sinais clínicos do trato urinário.
