- Abstract
Objective: To report clinical case of a choledochal cyst in a 49 years old female patient. Ambient: Service of General Surgery. Hospital Universitario de Los Andes. Mérida. Mérida State. Venezuela. Case report: formerly described patient arrived at consult with symptoms and laboratory tests related to acute pancraatitis of biliary origin. A retrograde pancreatocholangiography was practiced and revealed a type I choledochal cyst according to Todani's classification. The surgical finding was a 4 cm diameter supraduodenal dilatation in the choledocal duct. Total resectión of the cyst with hepatic-jejunal anastomosis was practiced. Conclusions: Choledochal cyst is a rare benign biliary disease mostly presenting during childhood. Adult presentation is rare and this patients with choledochal cyst have associated biliary problems such as the development of cholangiocarcinoma, liver cirrhosis with portal hypertension. They tend to present signs and symptoms similar to obstructive biliary disease. The best surgical option for these patients is a total cyst excision together with a Roux-en-Y hepaticojejunostomy. Patient outcome after surgery was good devoid of any complication.
