CUANTIFICACIÓN DE CITOCINAS PRO Y ANTIINFLAMATORIAS EN EL LÍQUIDO CEFALORRAQUÍDEO DE PACIENTES CON NEUROCISTICERCOSIS.

- Abstract
Objetivo: Evaluar la reacción inmunoinflamatoria central (líquido cefalorraquídeo, LCR) en pacientes afectados por diferentes tipos de neurocisticercosis. Antecedentes: La neurocisticercosis (NC) es una afección causada por la presencia de larvas de T. solium en el Sistema Nervioso Central. Dos mecanismos están implicados en las manifestaciones clínicas; el efecto de masa y la reacción inflamatoria asociada a la presencia del parásito. Evidencias muestran que la reacción inflamatoria es el principal factor implicado en la severidad de la enfermedad. Métodos: Se determinaron por ELISA citocinas pro y antiinflamatorias (IL-17A, IFN-γ, IL-1ß, IL-6, IL-10, IL-5, CCL-5, IL-4, TNF-a) en muestras de LCR de pacientes con NCC extraparenquimatosa (NC-EP, n=113) activa (n= 75) o inactiva (n=38) y parenquimatosa (NC-P, n=15) activa (n=6) e inactiva (n=9), así como en un grupo de pacientes control (pacientes neurológicos sin antecedentes de NC, n=27). Las diferencias de concentración entre los distintos grupos fueron evaluadas utilizando la prueba de Mann-Whitney. Protocolo: 63/14 Resultados: Las citocinas IL-5, IL-6, IL-10 presentaron concentraciones significativamente más elevadas en pacientes con NC-EP activa e inactiva vs. los pacientes control, mientras que IL-17A fue significativamente más alta en los pacientes control vs. pacientes con NC-EP activa. Los pacientes con NC-P activa o inactiva no presentaron diferencias significativas en la concentración de las citocinas comparado con los controles. Las citocinas proinflamatorias IL-17A y CCL-5 estuvieron más elevadas en pacientes con NC-EP inactiva vs pacientes con NC-EP activa, mientras que la citocina inmunomoduladora IL-10 fue más alta en los pacientes con NC-EP activa. (Figura) Conclusiones: Nuestros resultados confirman una reacción inmunoinflamatoria de relevancia en los pacientes con NC-EP vs. los controles. Comparando los pacientes con NC-EP activa vs. inactiva, la existencia de una mayor reacción inflamatoria en la NC-EP inactiva, y la presencia de una reacción inmunomoduladora en la NC-EP activa es muy interesante. Esto confirma la presencia de una desregulación de la reacción inflamatoria en los pacientes con NC-EP probablemente implicada en la evolución crónica y la pobre respuesta al tratamiento, y corrobora los resultados recientes obtenidos en la caracterización de la reacción inflamatoria sistémica.
