Portfolio clinics: can we improve access to and the educational value of workplace based assessments?

- Abstract
Introduction / background A November 2021 survey of IMT and ST4+ trainees at Liverpool University Hospitals revealed that only 7% found it 'easy' to obtain a case-based discussion (CBD) or mini-clinical evaluation exercise (CEX), and 76% found it 'difficult' or 'very difficult' to obtain an acute care assessment tool (ACAT). Moreover, 65% reported that workplace-based assessment (WPBA) feedback was 'not very' or 'not at all' valuable. Objective To improve the ease of obtaining and the educational value of WPBAs, by trialling (1) ambulatory emergency care (AEC) ACAT clinics, and (2) WPBA clinics. Methods (1) Inspiration was taken from the RCP's Never too busy to learn document.1 Meetings were held with the AEC leads to discuss logistics, then explanatory documents were distributed to trainees and AEC teams. Trainees attend a half-day session to clerk five patients in a 'live' ACAT. (2) A consultant's job plan was agreed to contain time for a weekly WPBA clinic. Results / benefits (1) Trainees are supernumerary for protected educational time. Twelve trainees have attended; four provided feedback. One could not complete an ACAT due to time constraints, the other three reported the clinic to be a 'very' or 'extremely valuable' learning opportunity. (2) Trainees select cases as best suits their educational needs. The retrospective nature provides protected, supported time to reflect on clinical care delivered during busy on-calls. Ten trainees have attended; five provided feedback; all reported this was a 'very' or 'extremely valuable' learning opportunity with very positive comments. Conclusions ACAT/WPBA clinics increase the educational value of portfolio assessments and are feasible in large, busy acute trusts.
