- Abstract
The article focuses on the number of patient safety incidents (PSIs) reported daily to the British National Patient Safety Agency (NPSA). About 2,000 PSIs a day are reported to the NPSA, while patients' chances of being harmed unintentionally during healthcare procedures are one in 300, odds that many people believe are too high. Healthcare staff are generally not alarmed about the number of PSIs each day. Indeed, many of them believe they are underreported.
