- Abstract
Introduction: Renal cell carcinoma (RCC) is associated with a particular biological propensity for vascular invasion, however the invasion of the tumor into the parasitic vessels is very unusual. We present a case of tumor thrombus invading into the parasitic vessel without renal vein thrombus in a case of metastatic RCC. Methods: 69 year old male presented with history of dull aching pain in the right flank associated with weakness, loss of weight and appetite for last 7-8 months. His routine work-up was normal. CT abdomen showed a heterogeneously enhancing mass (64x58mm) with contiguous infiltration into liver and posterior paranephric space. A dilated vessel could be seen arising from the mass and going into the pelvis upto the iliac bone. 99Tc bone scan revealed right iliac bone metastasis. Right sided radical nephrectomy was done. Intraoperatively there was dense fibroblastic reaction between liver and parietal wall. No renal vein thrombus. A large, dilated, tortuous lower polar parasitic vessel with a thrombus, reaching upto right iliac bone was seen. It was removed intact with the specimen. Histopathology revealed clear cell adenocarcinoma and tumor thrombus in the parasitic vessel.
