L'infermiere e il PICC Team: uno studio retrospettivo.

- Abstract
Introduction: the PICC is a device which allows central vascular access, Its diffusion has been growing in the last twenty years, thanks to the placement and management practicality, the cost effectiveness and the introduction of the PICC Team, within which the nurse has a central role. With this study we aim to evaluate the outcomes of the Perugua Hospital PICC Team in its first year of activity. Furthermore we evaluate the efficacy and efficiency in relation to the specialist nurse's new skills. Materials and Methods: we carried out a retrospective study on 307 devices implanted in Angiographic Unit from march 2016 to march 2017 without exclusions. We have also studied the PICC's clinical course by referring to clinical documents of each patient. The acquired data were elaborated by Software Stata14. Results: The 11,01% of devices have been repositioned during the hospital stay for complications such as bleeding 1,45%, infection 2,03%, occlusion 5,51%. No significant data were collected in relation to sex, age, lumens, ward, chemotherapy, antibiotics and endovenous therapy, except for parenteral nutrition which resulted into p=0,043. Discussion: a correct washing between drugs infusion can improve these results avoiding occlusion; moreover it's appropriate to reserve a lumen for parenteral nutrition infusions. It's also crucial to choose a proper timing for the device placement, which has often occurred to be not feasible due to arm's damages. Finally the Nurse should have a crucial role in the assessment of patient eligibility and devices should be implanted in a monitored environment. Conclusion: the high success rate of device placements performed by the PICC Team confirms nurse's new skills. This evidence can help promote the PICC's usage in the next future.
