ESTUDIO CLÍNICO COMPARATIVO DEL SULFATO FÉRRICO Y AGREGADO TRIÓXIDO MINERAL EN PULPOTOMÍAS DE MOLARES PRIMARIOS.

- Abstract
Resumen. El presente proyecto es un estudio clínico, en donde se evaluó la efectividad clínica y radiográfica del Sulfato Férrico y el Agregado Trióxido Mineral de pulpotomías en molares primarios realizadas en la clínica del Postgrado de Odontopediatría en la facultad de Odontología en la Universidad Autónoma deTamaulipas. Objetivos. -Comparar la efectividad del Sulfato Férrico y Agregado Trióxido Mineral como apósitos en el tratamiento de pulpotomías en molares primarios. -Evaluar clínica y radiográficamente la efectividad del Sulfato Férrico a 1, 3 y 6 meses en tratamientos de pulpotomías de molares primarios. -Evaluar clínica y radiográficamente la efectividad del MTA a 1, 3 y 6 meses en tratamientos de pulpotomías de molares primarios. -Identificar los principales signos y síntomas de fracaso clínico producidos con ambos materiales. -Registrar hallazgos radiográficos patológicos post-tratamiento con cada material. Materiales y métodos. La población del estudio estuvo conformada por pacientes pediátricos de ambos sexos, de 4 a 8 años de edad que acudieron a la clínica de Odontopediatría de la Universidad Autónoma de Tamaulipas en el período de noviembre del 2007 a mayo del 2008, la muestra fue tomada de pacientes pediátricos que tuvieran molares primarios con necesidad de tratamiento de pulpotomías y que reunieran los criterios de inclusión del presente estudio. El tamaño de la muestra fue de 26 molares primarios superiores e inferiores que se asignaron de manera aleatoria en los grupos de Sulfato Férrico (SF) y Agregado Trióxido Mineral (MTA). En una primera fase se realizo la anamnesis seguida de una toma radiográfica preoperatoria con la técnica de paralelismo ayudado con un colimador XCP (Denstply) y una revisión clínica para establecer el diagnóstico, y una vez establecido se explicaba detalladamente a los padres el procedimiento, quienes estuvieron de acuerdo, firmaron el consentimiento informado. Una vez autorizado el tratamiento, se prepararon los instrumentos y materiales necesarios. Después de administrar anestesia local o regional según el caso, se aisló con dique de hule el molar primario a tratar. Posteriormente se realizo un lavado con 1 ml de hipoclorito de sodio al 2.5% y un cepillo profiláctico con la finalidad de reducir la contaminación bacteriana de la cavidad. Se retiro toda la caries superficial antes de la exposición pulpar con ayuda de una fresa de carburo # 4 montada en una pieza de alta velocidad con sistema de refrigeración agua-aire; eliminada la caries se cambio la fresa # 4 por otra igual para establecer la comunicación de la cámara pulpar en el punto más cercano entre la cavidad y la cámara pulpar; para después cambiarla por una Endo-Z realizando movimientos de mesial a distal para retirar el techo y ampliar la cavidad pulpar. Se irrigo la pulpa cameral con 2 ml de hipoclorito de sodio al 2.5%. Posteriormente se amputaron los restos de pulpa coronal con una cucharilla de dentina # 17. Se irrigó la cavidad pulpar con solución fisiológica fría mediante una jeringa hipodérmica desechable de 10 ml, durante tres minutos. Posteriormente se colocaron torundas de algodón humedecidas con suero fisiológico en el sitio de la amputación por cinco minutos para lograr la hemostasia. Una vez obtenida la hemostasia se continuó el procedimiento con el material elegido para el tratamiento. Sulfato Férrico. Con la ayuda de unos apli-cadores desechables se coloco la solución al 15.5% en la entrada de cada conducto, dejándola en contacto con él por 15 segundos. Posteriormente se irrigo con suero fisiológico, después se secaron los conductos con torundas de algodón estériles, se coloco Óxido de Zinc y Eugenol, se cubrió con una mezcla de ionómero de vidrio para obturar por completo la apertura del acceso. Se tomo radiografía postoperatoria, para control, todos los instrumentos y materiales utilizados fueron previamente esterilizados en autoclave. Agregado Trióxido Mineral. Después de preparar la mezcla con tres partes de polvo ProRoot MR MTA y una parte de agua inyectable estéril de acuerdo con las instrucciones del fabricante, se coloco en el sitio de amputación una vez que se obtuvo la hemostasia. La pasta se llevo con una espátula estéril y se aplico sobre el muñón, después se condenso ligeramente con una torunda de algodón humedecida en solución fisiológica. A continuación se coloco una obturación temporal con ionómero de vidrio. Se tomo la radiografía post-tratamiento para control. En ambas técnicas la colocación de la corona de acero inoxidable como restauración final se realizo antes de siete días después del tratamiento. Las evaluaciones de seguimiento clínico y radiográfico fueron realizadas a uno, tres y seis meses post-tratamiento. Resultados. Los resultados fueron comparados entre ambos grupos con controles a uno, tres y seis meses post-tratamiento, recolectando los datos se obtuvieron porcentajes y se realizo la prueba estadística Chi-cuadrada con el nível de significancia de 0.05 utilizando el programa de Stat View 5.0. Dos pacientes tratados con Sulfato Férrico y un paciente tratado con MTA fueron excluidos del estudio debido a que no asistieron a la última evaluación. Un paciente fue excluido debido a que no se pudo controlar la hemorragia después de haber intentado la hemostasia dos veces. Se encontró que el grupo de Sulfato Férrico obtuvo un 100% clínico y 100% radiográfico. Para el Agregado Trióxido Mineral se reporto éxito clínico de 100% y 85.7% radiográfico, sin que exista una diferencia estadística entre ambos grupos con un valor (p=0.17). Encontramos que los hallazgos radiográficos patológicos en el grupo de MTA fue un total de 22.8%, estos hallazgos incrementaron en un período de tres a seis meses observando un 7.6% de reabsorción interna, un 7.6% presento reabsorción externa y un 7.6% de zonas radiolucidas perirradiculares e interradiculares. Discusión. La evaluación de la efectividad clínica y radiográfica del Sulfato Férrico y Agregado Trióxido Mineral en pulpotomías de órganos dentarios primarios ha sido evaluada con anterioridad por investigadores que han reportado diferentes porcentajes de éxito y fracaso tanto clínico como radiográfico. En el presente estudio nosotros obtuvimos resultados muy parejos con ambos materiales sin embargo encontramos una ligera ventaja del Sulfato Férrico sobre el MTA ya que como mencionan Bellet et al en 1995 y Jabbarifar et al en el 2004 en sus estudios un fracaso de tratamiento pulpares considerado cuando presenta uno o más de los siguientes signos o síntomas: dolor, movilidad patológica, presencia de fistula, rebasorción interna, reabsorción externa, zona radiolúcida periapical o en furca.5 El porcentaje registrado de efectividad considerando las evaluaciones de los hallazgos radiográfico patológicos fue para el Sulfato Férrico de 100% y del 85.7% para el grupo de Agregado Trióxido Mineral. Los hallazgos radiográficos que se encontraron fueron: reabsorción interna, reabsorción externa, presencia de zonas radiolúcidas perirradiculares e interradiculares. Los resultados coinciden parcialmente con los encontrados por Fei et al y los encontrados por Ibricevic al Jame en donde reportaron un éxito clínico de 1 00% igual que nosotros pero diferimos con los resultados de éxito radiográfico ya que ellos reportan 97% éxito radiográfico y nosotros 1 00% pero por otro lado los resultados difieren de las investigaciones que se han hecho sobre el MTA en pulpotomías de molares primarios.7,8 La obliteración del canal pulpar se encontró en dos pacientes (1 5.2%) que fueron tratados con MTA y en un paciente (7.6%) tratado con Sulfato Férrico pero no se consideraron como hallazgos patológicos al no presentar sintomatología alguna. Se observo la presencia de puente dentinario en dos molares (1 5.2%) tratados con MTA, a partir de los tres meses posteriores al tratamiento. Los fracasos en pulpotomías suelen atribuirse a la contaminación pulpar ocurrida por la microfiltración debido a que las coronas de acero no se colocan en la misma cita, o a un diagnóstico incorrecto durante la selección. Conclusión. Una vez terminado la investigación así como el análisis e interpretación de los datos resultantes, podemos concluir que: - La efectividad clínica del Sulfato Férrico fue de un 100% y un 100% de efectividad radiográfica. -La efectividad clínica del Agregado Trióxido Mineral fue de un 100% y 85.7% de efectividad radiográfica. -No se encontraron diferencias significativas entre el Sulfato Férrico y el Agregado Trióxido Mineral en un período de seis meses post-tratamiento. -Los hallazgos radiográficos patológicos tuvieron un incremento con el tiempo más severo en molares tratados con MTA en el período de tres a seis meses post-tratamiento. -Se observo la formación de puente dentinario en el 1 5.2% de los molares tratados con Agregado Trióxido Mineral a partir del tercer mes.
