Total Ca[sup 2+] handling for E-C coupling in the whole heart: An integrative analysis.

- Abstract
We assessed total Ca[sup 2+] handling (transport, flux) in excitation-contraction (E-C) coupling in a beating left ventricle (LV). We developed a new integrative analysis method that utilizes the internal Ca[sup 2+] recirculation fraction (RF), O[sub 2] consumption (<IMG>o[sub 2] ) for Ca[sup 2+] handling, and O[sub 2] cost of E[sub max] (contractility index) of the LV. We obtained the RF from the beat constant of the exponential decay component of the postextrasystolic potentiation, and the O[sub 2] cost of E[sub max] from <IMG>o[sub 2] measured at different E[sub max] . Our equation calculated the unknown total Ca[sup 2+] handling, futile Ca[sup 2+] cycling, and Ca[sup 2+] reactivity of E[sub max] from the RF and Ca[sup 2+] handling <IMG>o[sub 2] . The calculated total Ca[sup 2+] handling fell between 30 and 110 µmol/kg, depending on E[sub max] and pathological conditions. Our method also allowed an assessment of futile Ca[sup 2+] cycling and Ca[sup 2+] reactivity of E[sub max] in a beating LV. These data are not available using conventional methods. Our method can be used to better understand the pathophysiology of total Ca[sup 2+] handling in a beating heart.Key words: excitation-contraction coupling, myocardial Ca[sup 2+] , contractility, cardiac O[sub 2] consumption.Notre objectif a été d'évaluer le métabolisme Ca[sup 2+] total (transport, flux) durant le couplage excitation-contraction (E-C) dans le ventricule gauche (VG) en contraction. Nous avons mis au point une nouvelle méthode d'analyse intégrant la fraction de recirculation (FR) du Ca[sup 2+] intracellulaire, la consommation d'O[sub 2] (<IMG>o[sub 2] ) pour le métabolisme Ca[sup 2+] et la dépense d'O[sub 2] de l'E[sub max] (indice de contractilité) du VG. Nous avons obtenu la FR de la constante de temps (exprimée en unités de battements) de la décroissance exponentielle de la potentialisation post-extrasystolique, et la dépense d'O[sub 2] de l'E[sub max] de la <IMG>o[sub 2] mesurée à différents E[sub max] . Nous avons calculé le métabolisme Ca[sup 2+] total inconnu, le cycle futile du Ca[sup 2+] et la réactivité au Ca[sup 2+] de l'E[sub max] de la FR et de la <IMG>o[sub 2] du métabolisme Ca[sup 2+] . Le métabolisme Ca[sup 2+] total calculé a diminué de 30 à 110 µmol/kg selon l'E[sub max] et les conditions pathologiques. Nous avons pu aussi obtenir une bonne estimation du cycle futile du Ca[sup 2+] et de la réactivité au Ca[sup 2+] de l'E[sub max] dans le VG en contraction. Aucune méthode classique ne fournit ces informations.
