- Abstract
During the course of 1987, two major incidents occurred at the BP Oil (Grangemouth) Refinery and one at its associated crude oil Storage Terminal at Dalmeny. The three incidents were the subject of a Health and Safety (UK) Executive investigation. In the first incident, at Grangemouth, work commenced on a refinery line, without first ensuring that it was drained and empty. A valve was partially blocked and therefore passing. Because of this, the line contained a large quantity of liquid when the maintenance work began. This was released when the valve was lifted and ignited by the compressor. Further fuel to the fire was then added because another valve was passing liquid into an open line. This resulted in a major fire, the death of two workers and injury to a further two workers. There was a lack of risk assessment prior to the job. In the third incident, a fire occurred in a crude oil storage tank at the Dalmeny Terminal, resulting in the death of one worker from burns and asphyxiation. The central cause of this incident was unauthorised smoking by contractors in the storage tank.
