- Abstract
Negative pressure therapy on wounds (negative pressure wound therapy, NPWT) and surgical incisions (closed incision negative pressure therapy, ciNPT) have been identified as a beneficial factor for wound healing and the reduction of wound healing disorders in humans. Veterinary publications on negative pressure therapy have been focused on wound treatment rather than preventive use. The aim of this study was to evaluate the preventive use of negative pressure therapy on surgical incisions in horses (closed incision negative pressure therapy, ciNPT). In addition, factors effecting the short and long-term outcome of wound healing of midline incisions as well as return to athletic use after convalescence were evaluated. A population of 201 horses were included in this study. The surgical incisions of 57 horses were treated using sterile vacuum wound covers (Incision Management System, KCI Medizinprodukte GmbH, Wiesbaden). The control group consisted of 144 horses with surgical incisions treated with standard wound covers (dry, non-adhesive wound dressing). Factors such as breed, gender, age, weight, vital and laboratory parameters, intraoperative diagnosis, number of laparotomies performed, duration of surgery and anesthesia time and wound healing disorders were associated to outcome. Outcome parameters, including wound infection after the hospitalization, abdominal hernia, colic after convalescence, postoperative use and level of performance were collected from owners via standardized telephone questionnaire. No significant difference was found between laparotomy incisions treated with ciNPT compared to standard wound dressings. Significant effects were found between wound infection and heart rate on arrival in the clinic, internal body temperature on day 5 and 10 after surgery, white blood cell count on day 5 after surgery, diagnosis, duration of surgery and general anesthesia time. Horses with small intestinal disease in combination with colonic enterotomy and horses that required repeated laparotomy had a significantly higher risk of developing an abdominal hernia, compared to the rest of this population. Closed incision negative pressure therapy cannot be recommended for the reduction of wound healing disorders after midline laparotomy in horses.
