Prävention kardiovaskulärer Erkrankungen durch Blockade des Endocannabinoidsystems.

- Abstract
Zusammenfassung Risikofaktoren f�r die Entwicklung kardiovaskul�rer Erkrankungen, insbesondere des akuten Myokardinfarkts, sind Nikotinabusus, �bergewicht, wenig Bewegung, ung�nstige Ern�hrung, erh�hter Blutdruck, erh�hte Blutglucose bzw. Diabetes mellitus und eine Dyslipid�mie. Wird ein Risiko f�r kardiovaskul�re Morbidit�t in den n�chsten 10 Jahren von 5% (nach dem SCORE-Score) oder f�r nichtt�dliche kardiovaskul�re Ereignisse von 20% (PROCAM-Score) �berschritten oder liegt eine Gef��atherosklerose oder ein Diabetes mellitus Typ 2 vor, ist zur Pr�vention kardiovaskul�rer Erkrankungen der Einsatz einer (Poly-)Pharmakotherapie angezeigt, eine Lebensstil�nderung (Di�t, k�rperliche Bewegung) allein reicht zu diesem Zeitpunkt in der Regel nicht mehr aus. Eine neue Therapieoption, die eine Reihe kardiovaskul�rer Risikofaktoren gleichzeitig g�nstig beeinflusst, besteht in der Blockade des Endocannabinoidsystems. F�r die Substanz Rimonabant wurde in klinischen Studien nicht nur eine Reduktion des K�rpergewichts bzw. des Taillenumfangs, sondern auch ein Anstieg des HDL-Cholesterins sowie eine Absenkung der Triglyzeride nachgewiesen. Dar�ber hinaus wurde auch eine Verminderung des HbA1c bzw. der N�chternglucose beobachtet. Einschlie�lich seiner nachgewiesenen Wirksamkeit in der Raucherentw�hnung beeinflusst Rimonabant damit vier der acht vom American College of Cardiology und von der American Heart Association genannten relevanten Risikofaktoren zur Pr�vention des akuten Myokardinfarkts. Ein umfangreiches klinisches Studienprogramm zur Erforschung von Rimonabant im Rahmen des kardiovaskul�ren Risikomanagements wird gegenw�rtig durchgef�hrt. Die Substanz wird im Allgemeinen gut vertragen (h�ufigste Nebenwirkung: �belkeit). Die bislang ver�ffentlichten Studienergebnisse zeigen ein gutes Vertr�glichkeitsprofil und sind vielversprechend im Hinblick auf den zuk�nftigen Stellenwert von Rimonabant im Rahmen des kardiovaskul�ren Risikomanagements.
