FOOD-SPECIFIC IgG4 ANTIBODY-GUIDED ELIMINATION DIET IMPROVES HEARING THRESHOLD IN CHILDREN THAT HAVE OTITIS MEDIA WITH EFFUSION.

- Abstract
Otitis media with effusion (OME) is a very common ear disorder and the main cause of hearing impairment in children. Tl>e possible role of IgG4-related fo o d allergy in children with OME was determined. We investigated whether an 8-week food-specific IgG4-guided elimination diet influenced serum levels of lgG4, and compared hearing thresholds before and after the elimination diet. Thirty children with bilateral OME and 28 controls were enrolled fo r the hearing and blood tests of inhaled-specific IgE and food-specific IgG4. The families of the OME children then followed the elimination diet fo r 8 iveeks according to the food-specific IgG4 report, after which ear condition, hearing test and food-specific TgG4 were evaluated. There was no difference in the distribution of allergic disease between the two groups, however specific IgE of house dust mites, Timothy grass, mold, fungus, and ragweed were higher in the OME group than in the control. With regards to food-specific IgG4, the OME group had significantly higher antibody titers to egg white, salmon and egg yolk compared to the controls. After the 8-week food elimination diet, levels of 10 food-specific IgG4 including egg white, almond, carrot, potato, kiwi, soybean, wheat, casein, egg yolk and cod were decreased in the OME group. The hearing thresholds of OME children significantly improved both in the right and left ears after fo o d elimination diet. Nine cases with middle ear effusions still had persistent middle ear effusion after fo o d elimination diet and received tympanostomy tube insertion. For children with OME, blood tests fo r allergens to evaluate whether they have any type of combined allergic diseases are suggested. Except fo r specific IgE, food-specific IgG4 can also serve as a reference. An 8-week IgG4- guidedfood elimination diet can reduce IgG4 titers and improve the hearing threshold of patients with OME.
