- Abstract
This article provides an overview of device innovation in interventional cardiology and clinical trials in cardiovascular prevention. It emphasizes the importance of research in Europe in driving cardiovascular device innovation, including percutaneous coronary intervention, cardiac imaging, transcatheter heart valve implantation, and device therapy for cardiac arrhythmias and heart failure. The article also discusses the NOTION trial, which compared transcatheter aortic valve implantation (TAVI) with surgical aortic valve replacement (SAVR) and found no significant difference in major clinical outcomes after 10 years. Additionally, the article explores the impact of exercise on cardiovascular disease prevention and the comparative efficacy of different types of exercise on cardiovascular risk profiles. It concludes by discussing the management of cardiovascular disease before non-cardiac surgery, particularly in relation to patients receiving renin-angiotensin system inhibitors. The article suggests that guidelines on the use of these inhibitors before surgery should be re-evaluated. Another article examines the long-term effects of different blood pressure treatments on cardiovascular outcomes. The study found that systolic blood pressure variability (BPV) was a strong predictor of cardiovascular events, and amlodipine-based treatment was associated with a reduced risk of stroke, cardiovascular events, and atrial fibrillation compared to atenolol-based treatment. The authors recommend considering BPV as an additional modifiable risk factor in clinical practice. Lastly, a post-hoc analysis of a trial on icosapent ethyl in patients with recent acute
