- Abstract
Acute myocardial infarction with ST-segment elevation (AMI with ST supra) in pregnant women is a complex medical challenge, in which the need for specialized care is crucial to ensure the health of both the mother and the fetus. This summary explores the essential aspects of the clinical treatment of this condition, based on a systematic literature review. Objective: This systematic review aims to examine the clinical treatment protocols adopted for acute myocardial infarction with ST supra in pregnant women. Methodology: The literature review was conducted based on the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyzes) protocol. The PubMed, Scielo and Web of Science databases were consulted to identify relevant scientific papers, studies and books published by September 2021. The descriptors used were: "acute myocardial infarction", "ST segment over-leveling", "pregnant women", "clinical treatment" and "care protocols". Results: 15 articles were selected. The analysis of the selected studies revealed a multidisciplinary approach as standard of care for pregnant women with AMI with ST supra. Continuous cardiac monitoring was emphasized, along with assessment of maternal and fetal vital signs. Drug treatment includes the use of antiplatelet agents, anticoagulants and analgesics, adapted to maternal needs and considering potential risks to the fetus. The decision to carry out invasive interventions, such as coronary angioplasty, was guided by the seriousness of the infarction, the stage of pregnancy and an assessment of the risks and benefits. Collaboration between cardiologists, obstetricians and anesthetists was highlighted as essential for informed decision-making. Conclusion: The clinical treatment of heart attack with ST supra in pregnant women involves a complex and individualized approach, considering the potential risks for both mother and fetus. Multidisciplinary strategy, close monitoring and adaptation of drug therapy are key elements to optimize clinical outcomes. Decision-making on invasive interventions should be based on a careful assessment of the risks and benefits, taking into account the specific clinical situation. This review highlights the importance of ongoing studies and research to further enhance therapeutic approaches and improve outcomes for pregnant women with AMI with ST supra.
