- Abstract
Heart failure represents a progressive condition characterised by a decrease in the pump function of the heart. This disease affects 4.7% of the Romanian population with the age above 35 years old. The aim of this medical literature review is to identify the main objectives and the necessary interventions for an appropriate management of patients with advanced heart failure and palliative needs. Materials and method. The evaluation of 8 articles which brought information about the detection of the prognostic factors, the interventions for symptoms management, the evaluation of end-of-life state, the family support and the role of the multidisciplinary team. Results. The objectives of the caring process depend, on one hand, on the stage of the disease and, on the other hand, on the patient's life and living conditions. These items are in a continuous change, therefore the communication with the patient is essential for adapting the patient's expectations and the caring process objectives to the patient's condition. The initiation of palliative care and the detection of the prognostic factors are important elements for the care management of the patient with heart failure. The symptoms management has to combine both pharmacologic and nonpharmacologic interventions. The end-of-life stage of these patients is a period with a great psychoemotional strain that requires certain interventions in order to improve the quality of life and to reduce hospitalization. For achieving these goals, the professionals must involve and educate the family members on what caring for a palliative patient means, whether the patient is at home or in a palliative care bed unit. Conclusions. Taking care of a heart failure patient with palliative needs is a common situation for the general practitioner. In order to provide a proper care to this type of patients, the general practitioner needs a management plan based on specific and efficient interventions with positive results.
