Outcome of Post Coronary Angioplasty Patients in a Predominant Malay Population.

- Abstract
Objectives: In Malaysia, cardiovascular disease accounts for 20-25% of all deaths from 2000 to 2005. Majority of the population are of Malay race. Patients with acute coronary syndrome undergo percutaneous transluminal coronary angioplasty (PTCA) to improve outcome in STEMI and to alleviate symptoms in unstable angina and NSTEMI. Outcome of major adverse cardiovascular events (MACEs) amongst post PTCA patients in a predominant Malay population and the associated risk factors are still not widely known. Materials and methods: This was a prospective cohort study involving patients undergoing elective coronary angioplasty. Patients gave written consent and were followed up in the cardiology clinic for 6 months post procedure. Results: 240 patients were recruited and 5 dropped out during study. The study population were predominantly of the Malay race (91.9%). 21(8.9%) patients had MACEs post PTCA. 2 vessel residual disease (p = 0.004, adjusted OR 4.93 95% Cl 1.67-14.61) and Right Coronary Artery (RCA) involvement (p = 0.026, adjusted OR 5.53 95% Cl 1.22-24.96) were significantly associated with MACEs post PTCA. High HbA1C had significant association with MACEs (p = 0.011, adjusted OR 1.93, 95% Cl 1.16-3.20). Conclusion: The outcome of post angioplasty patients in our centre which has Malay predominance was good as incidence of MACES was low. Patients who have two vessel residual disease, RCA involvement and uncontrolled diabetes were significantly associated with MACEs and would benefit from earlier reangiographic studies and better glycaemic control.
