- Abstract
We thank Bernardo et al for their interest in our paper on utility of measurement of optic nerve sheath diameter (ONSD) with ultrasound in children with acute liver failure (ALF).[1] Their main concerns about the effectiveness of 2-D ultrasound (B-scan ultrasonography) as compared to A-scan ultrasonography are because of blooming effect and non-utilization of 30 SP 0 sp test which may help to differentiate increase in ONSD secondary to raised intracranial pressure from solid thickening of pial and arachnoidal sheaths.[2] Although we agree that the blooming effect is an important factor that may result in some error in measurement with B-scan ultrasound, we disagree that it is unreliable in measuring ONSD. Bernardo et al also agree that A-scan ultrasonography is not only difficult to perform but also not available on the general ultrasound machines for clinical use in the ICU. 2 De Bernardo M, Vitiello L, Capone M, Rosa N.
