Acquired Elastotic Hemangioma: A case report.

- Abstract
Acquired elastotic hemangioma (AEH) is a rare benign vascular proliferation that presents on sun-damaged skin of the forearms or neck in middle-aged and elderly females. Lesions clinically present as solitary erythematous plaques. The differential diagnosis includes actinic keratosis, squamous cell carcinoma, and basal cell carcinoma. Histologically, the differential diagnosis includes other hemangiomas, as well as Kaposi sarcoma and welldifferentiated angiosarcoma. To date, only 30 cases have been reported in the literature. We present an additional case of AEH in a 60-year-old female presenting as a solitary erythematous 2 cm plaque on the right extensor forearm. Histologically, sections demonstrated a band-like proliferation of capillaries parallel to the epidermis within the upper reticular dermis with plump endothelial cells often protruding into the lumen in a hobnail pattern. Surrounding solar elastosis was present. Immunohistochemistry showed expression of CD31 and CD34 and weak expression of D2-40. AEH is a rare distinctive clinicopathologic variant of hemangioma arising in sun-damaged skin, however awareness of this entity is important to avoid misdiagnosis.
