- Abstract
Introduction: Some epidemiological trends have been recently reported for pancreatic duct cell carcinoma in Western countries but no such data are available for the Polish population. Aim of study: The aim of this study was to analyse temporal trends in clinical and demographic characteristics at patients with pancreatic and ampullary cancer. Material and methods: Demographic and clinical variables of 1085 patients treated between 1972 and 2003 for cancer of the pancreas or the ampulla of Vater were analysed for possible time-related variations. Results: The mean age rose for both tumours, whereas the proportion of females increased only for pancreatic cancer. Distribution of stage groups in pancreatic cancer showed a reduction in stage IV with a concomitant increase in stages II and III. The resectability rate increased significantly from 9 to 31% in the last decade. Opposite trends were found for cancer of the ampulla of Vater, where the proportion of patients with unresectable tumours rose significantly from 0 to 10%. The overall median survival of pancreatic cancer patients increased significantly from 5.2 to 7.6 months with similar changes in the subgroups of resectable (from 14.3 to 20 months) and unresectable (from 5 to 5.9 months) disease. The observed changes were attributed to the increasingly role of systemic chemotherapy. Survival rates in patients with ampullary cancer remained stable over time with a median survival of 58.6 months. Conclusions: Prognosis of patients with pancreatic cancer have improved significantly over the last 30 years. Long-term outcome of ampullary cancers remains stable.
