- Abstract
The article discusses the effects of Sanford USD Medical Center's policy requiring baseline International Normalized Ratio (INR) for inpatients treated with warfarin in South Dakota. It notes that the policy requires pharmacists to review patient chart for the availability of baseline INR and ensure the appropriateness of prescribed warfarin dosage. Study suggests that pharmacists can implement a baseline-INR policy efficiently.
