- Abstract
Objetivo: En el bibliografía existente se ha descrito de forma sistemática una comorbilidad robusta y bidireccional entre el trastornos por déficit de atención con hiperactividad (TDAH) y los trastornos por consumo de sustancias psicoactivas (abuso o dependencia de alcohol o drogas) Método: Familiares de primer grado de un gran grupo de chicos derivados de pediatría y psiquiatría con TDAH (112 probandos, 385 familiares) y sin TDAH (105 probandos, 358 familiares) fueron ampliamente evaluados por examinadores ciegos con entrevistas de diagnóstico estructuradas. El análisis del riesgo familiar examinó los riesgos de TDAH en familiares de primer grado, trastorno por consumo de sustancias psicoactivas, dependencia del alcohol y dependencia de las drogas después de estratificar los probandos por la presencia o la ausencia de estos trastornos. Resultados: El TDAH en los probandos se asoció de forma sistematica con un riesgo significativo de TDAH en sus familiares. La drogodependencia en los probandos incrementó el riesgo de la drogodependencia en los familiares, con independencia del estado (presencia o ausencia) de TDAH, mientras que la dependencia de alcohol en los familiares sólo fue pronosticada por probandos con TDAH y dependencia del alcohol comórbida. Además, el TDAH en los probandos pronosticó la drogodependencia en los familiares, y la drogodependencia en los probandos de comparación aumentó el riesgo de TDAH en los familiares. Tanto la dependencia del alcohol como la drogodependencia se encontraron en las familias sin indicios de un riesgo común entre estos trastornos. Conclusiones: Los patrones del análisis de riesgo familiar sugieren que la asociación entre el TDAH y la drogodependencia es más coherente con la hipótesis de expresividad variable de un riesgo común entre estos trastornos, mientras que la asociación entre el TDAH y la dependencia del alcohol lo es con la hipótesis de la transmisión independiente de estos trastornos. Las conclusiones también apuntan a la especificidad de la transmisión de la dependencia del alcohol y las drogas.
