- Abstract
Of late, there has been much interest in tailoring additional ablation to patient-specific AF mechanisms, either elucidated by electroanatomic mapping to identify presumed localised drivers or through trigger-identification by isoproterenol administration after PVI. Despite extensive research, the pathophysiological mechanisms underlying initiation and maintenance of atrial fibrillation (AF) remain unclear, especially in patients with advanced persistent AF. Non-PV foci were induced in only one patient in whom AF was induced by firing from the posterior wall; this patient underwent box isolation and experienced no further AF. In this small pilot study of 12 patients with long-standing persistent AF, fractionation-guided ablation resulted in acute AF termination in 8 patients.
