Hipomineralização pode afetar terceiros molares?

- Abstract
Introdução: Os ameloblastos, células formadoras do esmalte dentário, podem sofrer influência de fatores sistêmicos ou locais nos períodos pré-natal, perinatal e durante a infância interferindo com o desenvolvimento normal desse tecido. Nesses períodos da vida, os indivíduos são mais suscetíveis à influência de fatores externos, que fizeram emergir uma condição denominada hipomineralização molar incisivo (MIH). Trata-se de um defeito de desenvolvimento qualitativo que afeta de um a quatro primeiros molares permanentes (PMP), frequentemente associado a alterações nos incisivos permanentes. Clinicamente, os dentes afetados exibem opacidades demarcadas, maiores que 1mm, variando em coloração e tamanho. O esmalte severamente afetado pode se desintegrar, expondo a dentina que é mais suscetível à cárie. Em crianças com PMP severamente afetados por MIH, uma das opções de tratamento é a exodontia, embora tal abordagem ainda seja considerada radical e possa dificultar o tratamento ortodôntico posteriormente. Para optar-se pela exodontia é preciso considerar a idade do paciente, padrão facial, presença de apinhamento ou espaçamentos dentários, presença de outras anomalias dentárias, gravidade da MIH, envolvimento pulpar, presença do germe do terceiro molar, possibilidade de restauração dos dentes e prognóstico em longo prazo, além do custo benefício. Entretanto, essa decisão implica na presença de segundos e terceiros molares hígidos. Objetivo: Este trabalho tem por objetivo descrever caso clínico de paciente com hipomineralização de terceiros molares. Descrição Do Caso: Paciente de 18 anos de idade, do sexo masculino buscou atendimento odontológico queixando- se de dor na região dos dentes 36 e 46 ao se alimentar. Ao exame clínico observou-se que terceiros molares inferiores estavam iniciando o processo eruptivo: 38 com as cúspides mesiais e 48 apenas a cúspide mésio-vestibular. As porções expostas dos dentes apresentavam cor amarela. Não havia espaço no arco para a completa erupção dentária e a terapia indicada foi a exodontia. Foi solicitada uma radiografia panorâmica, na qual se observou uma diferença de radiopacidade do esmalte dos terceiros molares inferiores quando comparados com os primeiros e segundos molares inferiores. Após a exodontia constatou-se que os dentes 28, 38 e 48 apresentavam as coroas com coloração amarelada e esmalte totalmente hipomineralizado e o 18 com 1/3 da coroa afetada. Avaliando-se o prontuário odontológico do paciente, foram localizadas três radiografias panorâmicas: aos 11 anos - na qual se observou os terceiros molares com 1/3 de coroa formada; 13 anos - terceiros molares com 2/3 de coroa formada e 15 anos - com a coroa totalmente formada. A investigação do prontuário médico do paciente mostrou apenas quatro episódios febris durante a provável época de mineralização de terceiros molares (aproximadamente de 10 a 14 anos), sem uso de antibióticos. Infelizmente o diagnóstico de MIH somente pode ser confirmado após a erupção completa da coroa, e, portanto, a decisão de exodontia de primeiros molares com MIH deve considerar a possibilidade de hipomineralização em terceiros molares. Conclusões: O diagnóstico precoce de dentes que apresentam MIH é fundamental para que o tratamento adequado seja realizado o mais rapidamente possível e danos minimizados. Deve ser considerada a possibilidade dos segundos e terceiros molares também estarem afetados.
