- Abstract
The thoracic vascular trauma is frequently fatal. Taking into account the patients that arrive to a hospital, a 15% of them requires thoracotomy and a 25% of them suffers an important vascular injury. The lack of knowledge of the anatomical structures that compound the pulmonary pedicle and its geometrical distribution will cause the potential damage of them, if the surgeon doesn't have an anatomical adequate management of the affected region. The aim of this study was to demonstrate the need of training for medical doctors residents of general surgery through the utilization of cadaveric models to get basic anatomical knowledge of the pulmonary pedicle and its relations, during the residency. Evaluation of 12 general surgery residents, regarding anatomical knowledge of the pulmonary pedicle and its relations, with cadaveric 10% formolized material, through checklists (May/2012). After a training period that included anatomical dissections in fifteen 10% formolized corpses and the identification of normal anatomical structures of the pulmonary pedicle and its variations (June/2012-September/2012); a new evaluation of the general surgery residents through checklists was made with cadaveric material (October/2012). During the first assessment, the mainly identified structures of the pulmonary peduncle were the pulmonary artery trunk, pulmonary artery bifurcation, right and left pulmonary artery and main bronchia, which were identified by 8 (66.66%) residents; and the most identified relations, were the descending aorta and the aortic arch by 9 (75%) residents. After training, its observed that the mainly identified structures of the pulmonary pedicle were the pulmonary artery trunk, bifurcation of the pulmonary artery, right and left pulmonary artery and right and left main bronchia, which were identified by the 12 (100%) residents; and the mainly identified relations were the descending aorta, aortic arch and the esophagus; which were identified by the 12 (100%) residents, the same for the anterior and posterior relations of the left pulmonary artery. The observation and anatomical dissection in cadaveric material offer an important teaching method and training for surgery residents to the recognition of the different anatomical structures of the region. The supervision and evaluation by surgeons through checklists is a way of improving the learning. It should be integrate in teaching programs.
