PROVÁVEL SARCOMA HISTIOCÍTICO HEMOFAGOCITÁRIO EM ROTTWEILER.

- Abstract
Fotomicrografia de medula óssea de paciente canino, fêmea, da raça Rottweiler, com quatro anos, previamente diagnosticada com sarcoma histiocítico (SH) em baço. Após a esplenectomia, a paciente passou a apresentar anemia progressiva, apesar da alta contagem de reticulócitos (314 x103/µL), e trombocitopenia, tornando-se dependente de transfusões sanguíneas para estabilização do quadro geral. Foi então realizado o mielograma para maiores esclarecimentos. As imagens demonstram: A. Espiculas medulares apresentando alta celularidade (em torno de 90%), composta por células de aspecto histiocítico, com citoplasma abundante, repleto de vacuolizações e, núcleo ovalado com cromatina frouxa e nucléolos evidentes. É possível observar anisocitose e anisocariose moderadas. A contagem total de monócitos e macrófagos foi de 14,2% (aumento de 400X). B. Agregado de células fusiformes, apresentando bordos citoplasmáticos pouco delimitados e anisocitose e anisocariose moderadas. Microscopicamente, o SH é conhecido por seu alto pleomorfismo, ocasionalmente apresentando células mais fusiformes. (aumento de 400X). C. Duas células histiocíticas fagocitando numerosos eritrócitos e precursores eritróides (aumento de 1000X). A avaliação da medula óssea revelou uma hiperplasia eritróide, porém associada à eritropoiese ineficiente. A associação do histórico, da bicitopenia observada em hemograma, do alto número de macrófagos e das figuras de eritrofagocitose permitiu a conclusão de uma síndrome hemofagocítica provavelmente secundária a um Sarcoma Histiocítico Hemofagocitário (SHH), possivelmente metástase do SH esplênico. Os distúrbios histiocíticos proliferativos em cães são um grupo raro de doenças e sua etiopatogenia ainda é desconhecida, dentre elas o SHH é o único que tem origem nos macrófagos e afeta mais comumente os Rottweilers, Berneses e Retrievers. Em decorrência da eritrofagocitose característica, a afecção tem comportamento agressivo e prognóstico desfavorável. O tutor optou por não realizar a eutanásia e a paciente foi submetida à terapia de suporte e administração de Lomustina. Sua sobrevida após o diagnóstico foi de quatro semanas.
