- Abstract
Introduction: Chronic skin and soft tissue infection usually occurs in immunocompromise patients. We review case series of lymphacutaneous disease due to sporotrichosis presented to orthopaedic clinic with the aim at looking into type of presentation and treatment of the disease. Case Report: We retrospectively reviewed 4 patients treated in orthopaedic clinic. They were between age 27 to 67 yrs old. All patients had multiple chronic skin ulcer of more than 2 months and did not have evidence of immunocompromise disease, however had prior skin injury while working outside the house (gardening, repairing bird cage). Site of injury underwent incomplete healing later indurated and ulcerate. It was later followed by development of multiple nodules (red or pink in colour) along lymphatic drainage with some of rupture producing pus and form chronic ulcers. The progressions occurred despite multiple episode of antibiotics treatment by general practitioner. There was no fever. White cell counts were at normal range. Immediate microscopic examination of the tissue did not find any organism however tissue culture for fungus resulted in the growth of Sporothrix schenchii Lesions in all patients healed with oral Iatriconazole 100mg daily for 2 to 3 months. Discussion & Conclusions: This study indicates the present of this organism in our community. All patients presented late due to misdiagnosis. Recognition of its clinical presentation is vital as microscopic examination is rarely conclusive and special fungal culture which may take up to 28 days before result is available.
