- Abstract
Background: The diagnosis and management of patients suffering from occlusal dysesthesia (OD) remains a major challenge for dentists and affected patients. Objectives: To present the results of a literature-based expert consensus intended to promote better understanding of OD and to facilitate the identification and management of affected patients. Methods: In 2018, electronic literature searches were carried out in PubMed, Cochrane Library, and Google Scholar as well Material und Methode: 2018 wurde eine elektronische Literatursuche ausgeführt in den Medien PubMed, Cochrane Library und Google Scholar. Zusätzlich wurde eine Handsuche in relevanten Zeitschriften ausgeführt, die in den aufgeführten Datenbanken nicht gelistet waren. Ergänzend wurde in den Quellenangaben der gefundenen Literatur nach weiteren relevanten Artikeln gesucht. Die Suchergebnisse wurden nach ihrem Evidenzgrad bewertet, ihr Inhalt geprüft und diskutiert. Das Ergebnis wird als Expertenkonsensus präsentiert. Ergebnis: Auf der Basis von 77 relevanten Quellen wird das aktuelle Wissen über das klinische Erscheinungsbild, die Epidemiologie, die Ätiologie, das diagnostische Vorgehen, die Differenzialdiagnosen und das Management der OD zusammengefasst. Schlußfolgerung: OD ist unabhängig von der Güte der Okklusion. Sie ist die Folge einer maladaptiven Signalverarbeitung. Die Betreuung der Patienten bezieht sich auf die Aufklärung der Patienten, deren Beratung, kognitive Verhaltenstherapie, unterstützender Einsatz von Medikamenten und unspezifische Maßnahmen. Irreversible okklusale Korrekturen und ausschließlich zahnärztliche Maßnahmen sollten vermieden werden.
