ARTHROSCOPIC REMOVAL OF A BULLET FROM THE HIP JOINT.

- Abstract
OBJECTIVE: It is rare for a bullet to remain in the hip following a gunshot injury and the removal of such a bullet by arthroscopy has been very rarely reported in literature. The aim of this study was to present such a rare case. MATERIAL AND METHOD: A 26-year-old male sustained gunshot injury in the left lumbar region 4 years previously. No surgical procedure was applied to the bullet that remained in his right hip joint. The patient was positioned supine with a support under the right hip. A high antero-lateral portal was opened between SIAS and the trochanter major by sending a surgical guide wire under fluoroscopy. An anterior neck image was obtained by sending the scope through this portal. Consequently, antero-inferior and antero-lateral portals were opened under fluoroscopy. These portals were used for visualization and shaving procedures. When the scope was in the inferior portal, the 9 mm. projectile was located, longitudinally impacted in the supero- lateral femoral neck with dense hypertrophic synovial tissue around. The bullet was retrieved by a clamp sent through the post-lateral portal. RESULTS: At the 9-month postoperative follow-up, abduction was 35°, internal and external rotation were 40° in a supine position, flexion was 120°. CONCLUSION: Open surgical procedures to the hip joint are difficult and risky due to the surrounding heavy muscles and important anatomical structures respectively. Apart from the complications that may occur after every operation, the only handicap of the hip arthroscopy method is the time and effort required for training.
