The article reports on the findings of a study which aims to identify and review the published literature on the operational definitions, incidence, and causes of medical dispensing errors in community and hospital pharmacy in the U.S., Great Britain, Spain, Brazil, and Australia. Dispensing errors identified by pharmacies include labeling medication with the incorrect directions and dispensing the wrong drug, quantity, or strength.
