- Abstract
Introduction. Diffuse large B-cell lymphomas (DLBCLs) are a heterogeneous group of aggressive Non-Hodgkin lymphomas with morphological characteristics of transformed Large B-cells. They have diffuse growth pattern and left without treatment will lead to a fatal end. The gastrointestinal tract is the most frequent location of the extranodal lymphomas and the stomach is affected in 60 to 75% of the cases. Clinical case. This is a case of a 75 y.o. woman admitted to the Clinic of Gastroenterology with upper dyspeptic complaints without associated symptoms. The laboratory tests prior to hospitalization show only increased level of β2 microglobulin and on the PET/CT a 20/15 mm zone with increased metabolic activity is seen on the lesser curvature of the stomach. The LDH and CA 19-9 done in the clinic are within the normal range. The upper gastroduodenoscopy reveals a giant 5-6 cm oval shaped ulcer at the distal part of the lesser curvature of the stomach. Histologically is confirmed an infiltrative growth of a DLBCL Non-Hodgkin type. The echoendoscopy (with linear transducer) reveals that the lesion affects all 5 layers of the stomach wall without adjacent lymphadenopathy and around the truncus celiacus. The abdominal CECT is normal. Following the recommendations of the oncologists the patient received chemotherapy of 4 consecutive treatments with RCVP. Conclusion: Approach to the patients with DLBCL of the stomach Is multidisciplinary. Chemotherapy is the first choice of treatment. PET/CT is the gold standard in the diagnostic and staging. EUS is more sensitive test than CT for detecting DLBCL of the stomach. There is a need of H. pylori testing and if test results are positive, an administration of a treatment for H. pylori eradication should be done.
