- Abstract
Objective: Reduced ejection fraction heart failure is one of the most important causes of cardiovascular morbidity and mortality. Patients can be decompensated even under medical treatment. Many metabolic and functional conditions may also contribute to worsening heart failure. The aim of the study was to determine whether the ratio of monocyte and high-density cholesterol, which is a part of routine laboratory measurements and used as of inflammation, atherosclerosis and a long-term prognostic marker fore some diseases, is a predictor for all-cause mortality at 1-year follow-up in patients who applied to the outpatient clinic under medical treatment without a history of hospitalization due to heart failure in the last 3 months. Material and Method: The patients were divided into 2 groups as with mortality (n=44) and without (n=219). The baseline demographic and laboratory characteristics of the patients at the time of admission were compared. Results: The ratio of monocyte and high-density cholesterol was 0.017±0.008 in the mortality group and 0.013±0.010 in the non-mortality group (p <00.1). Age, hemoglobin, uric acid, albumin, heart rate, creatinine, B-type natriuretic peptid, ischemic etiology and the ratio of monocyte and high-density cholesterol were significant in univariate analysis for mortality. In multivariate analysis, age, hemoglobin uric acid, ischemic etiology and monocyte/hidh density lipoprotein ratio were determined as independent predictors. (-2 Log Likelihood: 120,899 Nagelkerke R2: 0.426; p<0.001). The area under the receiver operating characteristic curve of the ratio of monocyte and high-density cholesterol for mortality prediction was 0.704 (0.620-0.788), the cut-off value was 0.0196, the sensitivity and specificity were 70.5%, and 73.6% (p<0.001). Conclusion: The ratio of monocyte and high-density cholesterol can be used as a practical and useful parameter for predicting long-term events in patients with compensated clinical heart failure.
