- Abstract
The objective of this study was to identify the opinion of medical internship students at the Federal University of Paraíba (UFPB) about their own knowledge about communicating bad news (BMC) to patients, focusing on the SPIKES protocol. This is an observational, cross-sectional study with a quantitative approach, involving 106 medical students in their 50s and 60s at the Centro de Ciências Médicas/UFPB. Data collection was carried out through the self-administration of a questionnaire containing demographic questions, about learning to communicate bad news during graduation and knowledge of the SPIKES protocol, the acronym in English that refers to the six recommended steps for communicating bad news. The sample showed percentage equivalence regarding gender, age between 21 and 40 years (mean: 25.6; standard deviation: 3.7 years); 71.7% single and 55.7% white (55.7%). It was observed that 83.9% revealed having received guidance on BCM, most before the internship, 34.9% considered their BCM ability to be good and 47.2%, regular. The main difficulty mentioned in the CMN was being honest without taking away the patient's hope (40.6%) while 64.2% reported feeling not very comfortable in this type of communication. It was found that 65.1% of the sample knew the SPIKES, 44.5% considered it practical, and the most difficult item to apply was referred to as recognizing how much the patient would like to know (29.2%). It was concluded that most students were aware of the existence and content of a structured protocol for communicating bad news, in addition to considering it a practical strategy in their future experience as a physician, but students do not seem to feel confident and secure in practicing the protocol. CMN and more often female students reported feeling uncomfortable in its execution.
