Paradoxical nail psoriasis induced by the IL-17A inhibitor ixekizumab in palmoplantar pustulosis Paradoxical psoriasis, defined as the onset of new psoriatic lesions or psoriatic lesions accentuated by a treatment, is occasionally observed under tumor necrosis factor (TNF)- blocking therapies.[1] Here, we report two cases of paradoxical nail psoriasis induced by the IL-17A inhibitor ixekizumab in patients treated for palmoplantar pustulosis. Improvement after risankizumab (b) gl The second patient was a 48-year-old woman with an 11-year history of severe PPP, plus plaque-type psoriasis on her elbows and mild nail psoriasis with distal onycholysis on two toe nails.
