NIVELES DE VITAMINA D EN PACIENTES CON SÍNDROME DE GUILLAIN-BARRÉ.

- Abstract
Introducción: se ha reportado alta frecuencia de vitamina D sérica en rangos no-suficientes en pacientes con enfermedades autoinmunes o inflamatorias del sistema nervioso; en el síndrome de Guillain-Barré (SGB) existe poca información. Objetivo: describir los niveles séricos de vitamina D en pacientes con SGB y sus características clínicas y electrofisiológicas. Material y métodos: estudio observacional prospectivo de pacientes consecutivos con diagnóstico de SGB por criterios de Ausbury durante un año, en un centro. De los pacientes incluidos obtuvimos características clínicas y paraclínicas; realizamos la determinación de niveles séricos de vitamina D (ng/mL) al ingreso y fueron clasificados de acuerdo con los niveles de vitamina D: suficientes >30ng/mL, insuficientes 20-30 ng/mL y deficientes <20ng/mL. Pobre pronóstico fue considerado como marcha no independiente a tres meses de seguimiento. Resultados: Se incluyeron un total de 56 pacientes, 85.7% género masculino, edad 46.1±15 años, índice de masa corporal 27.2±4.2 kg/m2, MRC score al ingreso 30±16.6 puntos. La variante electrofisiológica más frecuente fue AIDP 50%. Los niveles de vitamina D de la población fueron de 19.4±8 ng/mL, clasificándose en valores: suficientes 9% (IC95% 01-19%), insuficientes 32% (IC95% 20-45%) y deficientes 59% (IC95% 46-72%). En el análisis de correlación no hubo diferencias significativas entre niveles de vitamina D con: edad, puntuación MRC score, escalas EGOS modificada y EGRIS al ingreso. En el análisis comparativo entre los grupos de niveles de vitamina D, el antecedente de diarrea presentó diferencia significativa (p=0.042), pobre pronóstico no fue significativo (p=0.28). Conclusión: Se observó que el 91% de los pacientes diagnosticados con síndrome de Guillain-Barré presentan niveles de vitamina D séricos en rangos no-suficientes.
