Acute Oxalate Nephropathy after Organic Fruit Juice Ingestion.

- Abstract
Introduction: Secondary oxalosis is a rare cause of acute kidney injury. Elevated oxalate levels occur either due to an increase in intestinal absorption, endogenous production or excess in dietary oxalate intake. Design: A retrospective case report. Materials and Methods: A retrospective review of all patients diagnosed with biopsy proven oxalate nephropathy at our institution over the last five year period. Results: A 51 year old female with systemic lupus erythematosus referred with acute kidney injury was diagnosed with both lupus nephritis and oxalate nephropathy on renal biopsy. On further review, she had been taking an organic fruit juice containing oxalate for her systemic lupus erythematosus. Cessation of the organic fruit juice and treatment of lupus nephritis resulted in improvement in kidney function. Her lupus nephritis remains in remission on maintenance immunosuppressive therapy. Conclusion: Patients with pre existing renal disease have to be vigilant when selecting supplements or complementary medicine as they may cause more harm than good.
