- Abstract
Hyperventilation and excessively large (V)T (have been shown to)cause brain injury and lung injury, respectively, and the avoidance of these two complications is the major benefit of VG ventilation. There is little support for the stated hypothesis that: ''PSV+VG would be associated with improvement in oxygen and carbon dioxide exchange and with improvement in specific dynamic compliance.'' Available evidence supports the concept that VG is associated with more stable (V)T(, fewer excessively large tidal)volumes, and less hypocapnia. Assist control results in smaller fluctuations in blood pressure, lower respiratory rate and smaller, less variable (V)T(, when compared to SIMV.) Gas exchange has not been shown to be improved, nor would one expect it to be at equivalent ventilator settings. Less information is available for PSV, but this technique is closely related to assist control and likely to have similar effects.
